The inspector’s account

Licensing Program Analysts (LPA) Kesha Lewis arrived at the facility unannounced for the purpose of conducting a case management incident inspection regarding incident reports received dated 3/17/2025 for incident that happened in March 03/15/2025 and is ongoing. LPA explained purpose of visit to the administrator.

The incident reports were regarding R1 being sent to the hospital for a foot issue then then another time being sent out again for a fall that resulted in a broken femur. LPA reviewed incidents reports Resident records. Resident has moved to a skilled nursing facility on 04/22/2025. Facility will provided progress notes and care notes from Senior Doc mobile office that treated R1 during this time by COB on 04/25/2025. All reporting was done on time and to the required departments. This matter is still under investigation.

Per California Code of Regulations, Title 22 NO deficiencies were observed and are being cited during today's case management inspection.

An exit interview was conducted and a copy of this report was given.
