On 9-14-22 at 10:15am Licensing Program Analyst (LPA) Michael Bilger arrived unannounced to deliver amended documents related to complaint # 27-AS-20220826143712 visit conducted on 9-13-22. Additionally, LPA delivered copy of Civil Penalty and appeal rights paperwork related to case management conducted on 9-13-22. LPA met with Administrator Charles White and explained the purpose of the visit.
An exit interview was conducted with Charles White and a copy of this report was left with Charles.
