Licensing Program Analyst Bruce Jacobs conducted a case management visit at the facility on a request by the Facility Administrator. The facility is preparing to provide a 30 day notice for a resident and requested Licensing review the proposed notice. LPAS received a copy of the notice and reviewed the notice and Licensing requirement. LPA will also provide a copy to the Licensing Program Manager.
LPA also verify that signage is posted after a recent citation was issued to the facility.
No deficiencies were observed on this visit.
Exit interview held and a copy of this report given to Mr. Rai.
