Facility Evaluation Report
Licensing Program Analyst Albert Johnson arrived to conduct an unannounced case management visit. LPA met with staff and informed them that the reason for this case management visit is to make the facility aware of the fact that there is a eviction procedure that must be followed.
R1 was having falls and was losing the ability to ambulate without assistance. The program was requesting the move because of the inability to meet R1's needs. The program was in contact with the regional center and R1's POA/RP about the request to have R1 relocated.
The program going forward will submit a 3 day and a 30 day notice for any resident that the program feels need a higher level of care or for any other reason outlined in the advisory attached to this report.
No deficiencies were not issued today. However, a technical violation advisory was given.
