Facility Evaluation Report
Licensing Program Analyst (LPA) Albert Johnson conducted unannounced required one year annual inspection visit. LPA met with the program director and explained the purpose of the visit.
LPA and director inspected the physical plant including but not limited to the client classrooms; bathrooms, and outside area of the facility to ensure compliance with Title 22 regulations. Facility is an adult day program with a capacity of 140 with a current census of 116. Facility has seven classrooms and eight bathrooms. There is a separate gymnasium as well as a kitchen area not currently in use. No obstructions to fire exits noted. 5 staff and 10 client files were reviewed. Current First Aid and CPR certifications are in place for staff as well as fingerprint clearances. All other contents for files were in place.
Water temperature reads at 120*F in the bathroom and room temperature reads 75*F. Resident classrooms were sanitary and had the required furniture and furnishings. Smoke and carbon detectors were in good repair. Fire extinguishers were last inspected on November 2024. Facility has an emergency food and water kit. Motor carrier certification was completed on 11/15/2023 with a current terminal rating of satisfactory. Staff had current First Aid/CPR certificates, All staff have criminal record clearance and are associated to the facility. LPA reviewed the following updated documents on today's date: LIC 308 - Designation of Administrator, LIC 610-D Emergency Disaster, and the Surety Bond. Per the California Code of Regulations, Title 22 no deficiencies were observed or cited.
