Facility Evaluation Report
Licensing Program Analyst,(LPA) Noel Wolf Petersen arrived unannounced on 10/8/25 to perform an annual inspection of Vibrant Living Care Homes 2 LLC, at 9:15 am. Met with Administrator Dorris Madrid and licensee James Sadorra to explain the purpose of the visit. The facility is seeking vendorization as a 4 capacity ARF, for clients aged 18-59 and presenting no restricted conditions at this time, accepting self injected diabetic.
The Physical Plant was inspected, including the client bedrooms, bathrooms, common areas, storage areas, exteriors and evacuation routes. The kitchen had adequate lockable storage for sharps, toxics, and medications. There is adequate food for 2 days perishable, 7 days non perishable, and 3 days emergency. The facility is clean, traffic areas are well lit and unobstructed. the bedrooms have required furniture and furnishings including bed encasements. The bathrooms have functional hardware and water delivered between 105-120*F. Air temp is between 68-85*F. Fire extinguisher is dated 7/4/25. The smoke and CO alarms are functional. Posters are present and of the correct dimension. Exterior and interior activity space is ample and free of trip hazards/pests. evacuation gate swings freely and latches closed. First aid kit contains required items.
There are no clients currently in care, no staff interviews were completed, no clients were interviewed, no client files were reviewed, and 1 sample staff file was inspected. Sample staff file contained appropritate documents, tb screening, firstaid/cpr and background requirements, training onboarding and continuing.
The postlicensing will include a review of those documents. Administer documents were reviewed including but not limited to, evacuation plan, infection control plan, surety bond, liability insurance, program design(refund policy, house rules), facility sketch, administer certificate, and facility license and found to be complete and up to date.
No citations were issued, a copy of the report was read and given to the licensee.
