Facility Evaluation Report

On 7-16-2025 at 1:45pm, Licensing Program Analyst (LPA) Michael Bilger arrived unannounced to conduct a case management regarding an incident reported on 5-15-2025. LPA met with senior program manager (S1) Shirly Ridoloso and explained the purpose of the visit. LPA reviewed incident report and conducted brief interview with S1. Based on interview and record review, on 5-15-2025 at approximately 1:35pm, resident1 (R1) was sitting in a recliner in manager's office and upon getting up, tripped over another peer. Based on interview, R1 fell lightly forward resulting in R1 lightly striking his head on the floor. R1 sustain a red mark above right eye which was treated by staff using standard first aid. Fall was observed by staff on duty. R1 returned home upon his request without further incident and injury. R1 returned to day program with no further incidents. R1's responsible party was notified of incident.

As a result of today's case management, no citations are issued. An exit interview was conducted with S1 and a copy of this report was provided.
