LPA Albert Johnson made an unannounced POC visit to the facility to verify correction of citations issued during the annual visit conducted on 4/08/2025 .
LPA toured the facility, reviewed email submitted invoice/permit number BP25-02925 for plans of correction observed that the deficiency cited on 4/08/2025 has not been cleared.
The facility is using the area and the accordion doors for one stall is broken and has been replaced with a curtain. The facility is meeting with their contractor on 5/23/2025 to finalize the plans and begin replacement/repair.
Exit interview conducted and a copy of this report was left at the facility.
