Facility Evaluation Report

On 8-7-2025 at 10:00am, the Regional Office conducted an informal meeting via Teams with licensee designee members to discuss recent issued citations and additional concerns. Present at the meeting were Licensing Program Manager (LPM) Liza King, Licensing Program Analyst (LPA) Michael Bilger, Telecare Representatives Christina Mance, Amanda Duggirala, and Serena Figeroa, Regional Center Representatives, Brian Bennett and Katina Richinson, Department of Developmental Services (DDS) representatives Catherine Knight and Sunni Polani, and Ombudsman Kathryn Thomas.

LPA Bilger and LPM King discussed with Telecare representatives the following citations issued within the last 12 months, and the associated plans of correction going forward:

On 10/16/2024, Licensee was cited by the Department based on a case management outcome regarding a medication error and care and supervision related to an improper use of a restraint. The following citations were issued: Type A – Section 80075(b) Health Related Services and Type B – Section 80078(a) Responsibility for Providing Care and Supervision. Additionally, civil penalties were issued due to repeat violation of these sections.
