Facility Evaluation Report

On 3-30-22 at 4:50pm, Licensing Program Analysts (LPAs) Michael Bilger and Maja Jensen conducted a case management visit relating to a complaint visit conducted for #27-AS-20220118104227. During an interview with R1 and facility observation it was determined that R1s emergency signal system was not functioning. LPA Jensen activated signal system at 2:29pm. No response was heard by staff and no indication on monitoring system was observed.

Based on LPAs observation, deficiencies are cited as a result of today's visit under Title 22, Division 6. An exit interview was conducted with Alma Whitted and a copy of this report was left with Alma. Appeal rights provided.
