Resident assaulted another resident resulting in questionable death.

On 8/2024, LPA Albert Johnson arrived unannounced to deliver findings for the above allegation.

The department investigated whether staff neglect resulted in the altercation between the two residents, which resulted in the death of the victim, The department does not have the evidence to support that the facility failed to adequately supervise the residents resulting in the altercation. Based on the department's report, this was an isolated incident between the two residents. There were no prior incident reports of the perpetrator being physically or verbally aggressive with anyone that would have prompted facility staff to provide increased supervision to him. Additionally, two staff were present at the time of the incident.
