Facility Evaluation Report
**This report was amended to correctly identify Anna Simms as a representative of VMRC, and add titles to Brian Bennet and Anna Simms**
On 6/15/26 a non compliance conference was held via Microsoft Teams. The purpose of the NCC was to discuss the complaints since licensure. Present at today’s NCC were the Sacramento South Regional Office Adult and Senior Staff, Regional Manager (RM) Stephenie Doub, Licensing Program Manager (LPM) Liza King, Licensing Program Analyst (LPA) Noel WolfPetersen, Valley Mountain Regional Representatives Director of Community Services Brian Bennet and Quality Assurance Program Manager Anna Simms, as well as Licensee and Administrator (LIC/ADM) Arlene Acuavera.
Upon review of a departmental report in a case mangement 5/20/26 it was substantiated that a lack of care and supervision 80078(a) had the effect of delayed medical attention to a client in the care of the facility. Licensee expressed that she did not agree with the specific timeline of the incident as outlined by the department or that neglect precipitated or exacerbated the event, but did agree that untimely medical attention was the result. The administrator's account provided that in November of 2025 a client was expressing pain on the morning of 17th and received medical attention on the evening of the 18th.
The recent incident was discussed in the context of preventing a potential similar incident in the future.
Several areas of improvement were identified: the training of the staff in the use of emergency services, the administrators planning for transport needs of the clients, the use of a backup administrator. These areas of improvement should be addressed in ammenda to the plan of operation sent in to the office by 6/29/26, as well as copies of LIC 500's showing administrator hours for both facilities and LIC 308's designating a backup administrator. The VMRC representatives asked for an updated organizational chart.
