Facility Evaluation Report

Licensing Program Analyst, LPA, Noel Wolf Petersen arrived unannounced to the facility to deliver an amended report to the facility. LPA Met with Emila bagnion By phone and explained the purpose of the visit, and was designated Dendoy "Lang" to sign the reports.

Amended report for 5/20/26, adding required language for the further review and fixing some formatting such that it fit a single page.

Physical inspection was conducted the facility is clean, and the traffic areas are unobstructed. interviewed resident, reported everything is fine.

A copy of the report was read and delivered to the staff, and emailed to the care home. exit interview was conducted.
