Facility Evaluation Report
Licensing Program Analyst (LPA) Albert Johnson conducted a health and safety check on this day. LPA met with Staff.
Five Residents have been relocated and are adjusting to their new environment. Today's Health and Safety check included food supply, physical plant and staffing. Based on this inspection/visit the facility is in compliance.
The remaining resident has been given assistance with relocation effort and has refused when offered to move to one of these facilities. Out of the ten care homes visited, three were willing to accept R1. As of today, those facilities are still waiting to know when he will be moving in. However, R1's POA has declined those options because they are looking for a care home in Stockton with a monthly rate of $2,700 .
No deficiencies were cited pursuant to Title 22 rules and regulations, Health and Safety Codes.
