Facility Evaluation Report

Licensing Program Analysts (LPA) Kesha Lewis arrived at the facility unannounced for the purpose of conducting a case management visit regarding an incident where a resident hit a staff member and another residents visiting family.

LPA Lewis talked with staff present and the licensee on the phone. The facility is taking steps to control R1's behavior. The licensee will email R1'S new care plan to LPA by 5/27/2024 COB.

Per California Code of Regulations, Title 22 no deficiencies were observed or cited during today's case management inspection.

An exit interview was conducted and a copy of this report was given.
