Facility Evaluation Report

Licensing Program Analyst, LPA, Noel Wolf Petersen arrived unannounced at 11:00am to conduct an annual inspection. LPA Met with licensee Cherryl Magsayo to explain the purpose of the visit. Rivmags Care Home LLC, vendorized through VMRC, is a 4 capacity ARF for adults aged 18-59 m/f with a current census of 4. No restricted conditions, 1 diabedes client whos got oral medication.

The physical plant was inspected including the kitchen, common areas, storage areas, resident bedrooms and resident bathrooms, exteriors and evacuation routes. Facility is clean and and traffic areas are unobstructed. Common areas are clean and and there are space for activities. The resident bathrooms have functional hardware and the water temperature was 114*F. the resident bedrooms have the required furnishings. The storage areas have adequate storage for sharps, toxics, and medications. The exteriors have no trip hazzards and have appropriately sized walkways to the emergency exit; the exit is in good repair. the required posters are present including the resident rights, ombudsman, federal work requirements, facility sketch with evacuation route. The fire extinguisher was dated 5/27/25.

Client and Staff Records Review. 5 of 5 staff records reviewed for fingerprinting doj/fbi clearance, Cpr and first aid training, as well as continunuing education. 2 of 5 staff interviewed. 0 clients interviewed. 2 client files reviewed, for PnI, Mar, signed admission agreements,IPps, recent medical visits, and rececent behavior. Files are present and up to date.

Administrator Documents reviewed, secratary of state, surtey bond, liability insuance, gaurdian assoication, facility license, facility sketch, Infection control plan, and Evacuation control Plan. Files Present and up to date.

No defficies, cited, a copy of the report was read and given to the administrator. Exit interview conducted.
