The inspector’s account

on 7/15/25 at 2:00 PM Licensing Program Analyst(LPA) Noel Wolf Petersen arrived unannounced to conduct a annual inspection, he met with staff Marilou and administrator elizabeth rivera and explained the purpose of the visit. Rivera's Care Home is a 4 bedroom ARF with a current census of 4 ambulatory adults aged 18-59. One diabetic individual is at the facility, the administrator has an exception and he is able to administer his own injections and the facility is monitoring that ability continues through time.

The physical Plant was inspected including but not limited to the client bedrooms, bathrooms, common areas, storage areas, the exteriors, and evacuation routes. the facility is clean and traffic areas are well lit and unobstructed. Client bedrooms are have all required furniture and furnishings. the bathrooms are equipped with functional hardware, water temperature was measured within the required range 105-120*F. storage areas for toxics, sharps, and medication are lockable. The exteriors and fire escape gate are in good repair. The required posters are presnt including the federal work rights, the ombudsman poster, the client rights poster, and the facility sketch with evacuation routes. The fire extinguisher is dated 4/8/25. The MAR was checked for all four clients, several documentation errors of different types were present: drugs administered to the client were not documented, 3 medications had no start or received date. 2 had the incorrect start date. 1 clients bubble packs were given to the client in reverse order. The administrator was receptive to ensuring staff were trained in proper documentation of perscribed medication.

4 client files were reviewed for documents including but not limited to the signed admission agrement, recent medical assessment, preappraisal, and inital medical clearance. documents were found to be up to date and present.
