Facility Evaluation Report

Unannounced case management visit made out to this facility on 03/30/2026 by this Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility representative, Rachel Rae Santiago, who was briefly interviewed and was requested by this LPA to inform the facility designated Administrator, Rolando Santiago, that CCL was present at this time.

The facility designated Administrator, Rolando Santiago, was present and briefly interviewed at this time.

The purpose of this case management visit was to follow up on some potential issues that were recently brought to the attention of this LPA at this time:

Wood Flooring in need of repair/replacement

Tiles in common areas in need of repair/replacement

Missing window screens-others in need of repair/replacement

The following deficiencies were observed and cited on the following LIC 809-D pursuant to Title 22 Rules and Regulations, Health and Safety Codes.

Appeal Rights were printed and a version was given to the facility designated Administrator at this time.
