At 11:00am, LPA T. White arrived unannounced to conduct a case management visit. LPA met with Direct Support Staff, Iciewanna Jordan and Administrator, Muhammad "Ali" Qureshi. LPA explained the purpose of today’s inspection. LPA was allowed entry into the facility that is licensed to serve a total capacity of 5 clients.
Based on observation and interviews, LPA observed 5 clients and 3 staff members. LPA confirmed Staff #1 (S1) has not worked at the facility since December 2020.
Exit interview conducted with Administrator and a copy of report given.
