The inspector’s account

On 11-4-2025 at 10:30am, Licensing Program Analyst (LPA) Michael Bilger arrived unannounced to conduct a case management regarding a reported absence without leave (AWOL). LPA met with Assistant Administrator Christina Huerta and explained the purpose of the visit. LPA reviewed incident report with Assistant Administrator during today's visit. Based on review of incident report, it was determined that on 10-9-2025, resident1 (R1) became agitated upon returning home from day program, and proceeded to AWOL from facility. Staff on duty, attempted to locate R1 but was unsuccessful. R1 notified 9-1-1 to file missing persons report. Approximately 30 minutes later, staff on duty received notification from local law enforcement that R1 was at a local movie theater. While at move theater, R1 called ambulance to report he was suicidal. R1 was then taken to local hospital for evaluation. Based on interview conducted, it was revealed that R1 attempted to leave his day program prior to returning home, and AWOL'ing from facility.

As a result of this case management, it was determined that R1 was absent without supervision for approximately 30 minutes. As a result, citation is issued under Title 22, Division 6 and noted on LIC 809D. An immediate civil penalty in the amount of $500 is issued in addition to citation due to absence of supervision. An exit interview was conducted with assistant administrator and a copy of this report was provided.
