The inspector’s account

Licensing Program Analyst (LPA) Albert Johnson conducted an unannounced Case Management visit today at the facility. LPA was following -up on an incident dated 10/2/2025. R1 was seen at the ER for a chemical inhalation exposure.

During visit LPA Johnson reviewed records for R1, interviewed the Staff about events related to R1's service needs and the day of the incident. The records reviewed included ER discharge papers, case notes, day program service plan and the individual program plan for R1.

The records indicated that R1 was working off-site of the day program with a cleaning crew of six and two staff members. During the off-site work R1 was not supervised during the time while mixing toxins( bleach and ammonia) causing R1 to feel unwell and dizzy. R1 was transported to Dameron ER for evaluation and discharged.

Deficiencies were cited during this visit and civil penalty assessed.
