LPA Albert Johnson made an unannounced POC visit to the facility to verify correction of citation issued during the annual visit.

The following deficiencies, initially cited during a visit on 08/06/2025, have been cleared:

Section Cited: 80092.2(a) Date Due: 08/20/2025 Plan of Correction: Administrator shall submit an updated health care plan for R1 and R2. This shall be done by POC date 8/20/2025. Corrections: Cleared By Visit on 12/11/2025.

The department has received information regarding R1, R1 is now considered Non-ambulatory and this is R1's new baseline. R1 cannot transfer at all and cannot pull up using a grab bar, R1 is being transferred by an hoyer lift. The facility anticipates receiving R1 back with 24hr nurse support and the regional center will be providing an hoyer lyft. This is considered a prohibited health condition.

(a) In adult CCFs clients who require health services or have a health condition including, but not limited to, those specified below shall not be admitted or retained.

(3) Conditions that require 24-hour nursing care and/or monitoring.
