Facility Evaluation Report

LPA Albert Johnson made an unannounced POC visit to the facility to verify correction of citations issued during the case management annual visit conducted on 4/24/2025 .

LPA toured the facility, reviewed email submitted invoice/permit number BP25-03258 for plans of correction observed that the deficiency cited on 4/24/2025 has been cleared.

The facility will be using the bathroom located next to the office in the unattached area by the garage. The facility has installed grab bars as well as non-slip mat. This area will be used during construction/ repair of the main bathroom. The facility is meeting with their contractor on 5/26/2025 to finalize the plans and begin replacement/repair.

Exit interview conducted and a copy of this report was left at the facility.
