Facility Evaluation Report
On today’s date, Licensing Program Analyst (LPA) conducted an unannounced visit to the facility to follow up on an incident involving R1. The following information was obtained through interviews with staff and review of available documentation.
R1 requested his marijuana pipe from Direct Support Professional (DSP). DSP informed R1 that she did not know where the item was located. R1 then went to his room and returned to the common area shortly after. Staff reported that R1 appeared agitated and began yelling and using inappropriate language toward DSPs.
Staff stated that R1 attempted to swing at DSPs but did not make contact. R1 then picked up his cane and struck DSP in the head. DSP attempted to safely remove the cane; however, R1 resisted and continued to display aggressive behavior. DSPs implemented CPI safety interventions in response to R1’s escalating behavior.
During the incident, staff reported that R1 scratched and bit DSPs while they attempted to maintain safety. LVN contacted 911, and the facility administrator was notified. Staff stated that R1 eventually calmed down and returned to his room. Law enforcement responded to the facility and spoke with R1 regarding the incident.
No additional concerns were reported at the time of the visit.
