Facility Evaluation Report

Licensing Program Analyst (LPA) Albert Johnson conducted an unannounced annual visit on this date. Currently there are no residents in care.

The facility is a single story structure with wheelchair accessibility. The facility is licensed for 6 clients of which all can be non-ambulatory.

Hot water temperature is within range of 105 to 120 degrees. Fire extinguishers are current and in compliance with fire safety. Carbon dioxide monitor present. First aid kit was checked and is complete.

No deficiencies were cited as a result of today's visit. Advisories given for the backyard debris.

An exit interview was conducted and left with the facility.
