Facility Evaluation Report
On 7/13/23 at approximately 10am Licensing Program Analyst (LPA) Maja Jensen arrived at facility unannounced to conduct a required 1 year annual inspection. LPA Jensen met with Administrator Richanda Crandall and explained the purpose of today's visit.
The facility is a single story structure where a Day Program is conducted. LPA Jensen toured the physical plant including the recreation room, kitchen, bathrooms, quiet room, computer lab, fitness area and offices. The facility was observed to be clean, sanitary and in good repair with adequate furniture and lighting throughout. All outdoor and indoor passageways were observed to be clear of potential hazards and free of obstruction. The Day Program was in session and consumers were rehearsing for a talent show. There is a stage in the main room for performing arts.
Toxins and sharps were observed to be locked and inaccessible to residents in care. The facility does not currently administer medication. The facility conducts cooking classes and all food is dated. There was no expired food on the premises. The Emergency Disaster Plan was last updated on 5/17/23 and is in compliance. The facility conducts regular fire drills. The first aid kit was observed to be complete with including but limited to scissors, tweezers, thermometer and manual. The facility maintains an emergency supply of PPE and COVID tests. The smoke detectors and carbon monoxide detectors are in compliance. The fire extinguisher was last serviced in April of 2023 and is in compliance. The facility was a comfortable temperature and the bathroom water temperature was measured at 113.4 degrees Fahrenheit which falls within the required range of 105-120 degrees Fahrenheit. There are grab bars in the bathroom by the toilets.
The Inspection tool was used during the course of this visit. The facility employees 4 staff and 2 specialists. LPA Jensen reviewed 4 of 4 staff files and found them to be complete. Staff CPR certifications are all current. The facility conducts and documents training in excess of 8 hours per year per staff member.
