Facility Evaluation Report
Licensing Program Analyst (LPA) Albert Johnson arrived unannounced and met with Program Director, to follow-up on a incident report /case management visit.
This visit is in response to a SOC 341 Report of Suspected Dependant Adult/Elder Abuse that was submitted to the department by the Day program. The report indicates that a staff, S1, was alleged to have possibly inappropriately touched a resident at the program. This was reported on 7/11/2025
The program reported the alleged incident to all required agencies and did an internal investigation assisted by Stockton Police Department Officer Sahagun report number #25-20778. It was determined that S1 touched R1 on the shoulder and poked R1's chest to get R1's attention and there was no inappropriateness related to the touching.
R1, five days(7/16/2025) later made another allegation of inappropriate touching this time it was reported to be a peer. The program reported the alleged incident to all required agencies and did an internal investigation assisted by Stockton Police Department Officer Donaldson report number #2521391. It was determined that R1 and R2 were horseplaying about a year ago and there have been no other incidents with R2.
No deficiencies are being cited during today's visit.
