Facility Evaluation Report
Unannounced Plan of Correction visit made out to this facility on 09/04/2025 by Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility staff person Filomena Tetro. A brief interview was conducted with the facility staff person at this time. This LPA requested that the facility staff person go ahead and contact the facility designated Administrator, Brandon Rose, to inform him that CCL was present at this time.
The purpose of this visit was to follow up on the deficiencies that were cited from a prior annual visit conducted on 02/19/2025. This visit was to follow up on the Plans of Correction that were due.
The following deficiencies were observed and cited on 02/19/2025:
An Infection Control Plan shall be developed by the licensee and shall be included in the Plan of Operation required by Section 87208. (1) The Infection Control Plan shall include all of the following: (C) An Infection Control Training Plan. 3. The description of initial and ongoing training shall address the requirements of subsections (a), (b) and (d).
In addition to paragraph (1), training requirements shall also include an additional 20 hours annually, eight hours of which shall be dementia care training, as required by subdivision (a) of Section 1569.626, and four hours of which shall be specific to postural supports, restricted health conditions, and hospice care, as required by subdivision (a) of Section 1569.696. This training shall be administered on the job, or in a classroom setting, or both, and may include online training.
