Facility Evaluation Report

Licensing Program Analysts (LPA) Kesha Lewis arrived at the facility unannounced for the purpose of conducting a case management incident inspection regarding incident reports received dated 5/16/2025. LPA explained purpose of visit to the administrator.

The incident reports were regarding a death of a resident (R1). LPA reviewed incidents reports Resident records facility had previously emailed LPA Lewis the documentation. The department requested a copy of R1'S death certificate. No further follow up is needed at this time regarding this incident, all reporting was done on time and to the required departments.

Per California Code of Regulations, Title 22 NO deficiencies were observed and are being cited during today's case management inspection.

An exit interview was conducted, and a copy of this report was given.
