Licensing Program Analyst (LPA) Albert Johnson arrived unannounced to conduct a case management visit. LPA met with Administrator/Licensee Robert Felix and explained the purpose of the visit. Census: 5.

Community Care Licensing (CCL) received a LIC 624 reporting that on 05/04/2026, New Hope Guest Home experienced a medical emergency involving Resident 1 (R1). At the time of the incident, five clients were present in the facility. The facility notified all required agencies and obtained a police report number related to the incident.

LPA reviewed R1’s records, including the medication administration log and charting. LPA also interviewed staff. Documentation and interviews confirmed that R1 has a known history of heart ‑ related complications, which were being monitored by R1’s primary care physician. Records show the physician conducted a medication review and general physical examination in March 2026. Based on the information obtained, no deficiencies were cited pursuant to Title 22 regulations and Health and Safety Code.

An exit interview was conducted with Administrator/Licensee Robert Felix, and a copy of the report was provided.
