The inspector’s account

Licensing Program Analyst (LPA) Albert Johnson arrived unannounced to conduct an annual inspection. LPA met with Staff and explained the purpose of the visit. This is a level 4I home vendored with Valley Mountain Regional Center.

LPA inspected the physical plant including but not limited to the kitchen, dining room, resident bedrooms; resident bathrooms, medication room, laundry room, activity room, and outside courtyards. LPA observed sufficient furniture and lighting throughout the facility. LPA observed sufficient seven day non-perishable and two day perishable food supplies. Hot water temperature was measured at 117.5 degrees Fahrenheit in resident bathroom sink, which is within the required range of 105 to 120 degrees.

Fire extinguishers and smoke detectors are current and in compliance with fire safety. Fire drill was conducted on 10/2/2025. LPA observed centrally stored medications are kept locked and inaccessible to residents. During the medication review LPA and Administrator observed to medication errors for R1. Medication was found on the mantel by the front door by LPA and there was a medication to be destroyed for R1 that was marked as given by Staff on 10/05/2025. The medication found was also marked as given on 10/20/2025. LPA reviewed 4 resident and 2 staff files, including criminal record clearances and the P and I for all residents in care. All staff are fingerprint cleared and associated to the facility. First aid kit was checked and is complete.

Per California Code of Regulations, Title 22 Division 6, Chapter 8, deficiencies were cited during this visit. Copy of report given at the conclusion of the visit.
