Facility Evaluation Report
Licensing Program Analyst, LPA, Noel Wolf Petersen arrived unannounced to conduct a annual inspection visit, met with administrator Victoria Obuehi-Nazambi to explain the purpose of the visit.
Physical inspection was complete, included but not limited to the kitchen, bedrooms, bathrooms, common areas, storage areas, the exterior, and the evacuation route. Facility is clean, traffic areas are well lit and unobstructed. Kitchen has adequate food supply, 2 days perishable and 7 days non perishable. bedrooms have required furniture and furnishings, including mattress encasements. bathrooms have hardware in good repair. common areas and exteriors are clean, LPA pointed out some fruits on the ground and weeds but free of trip hazards. The evacuation route gate is a little sticky, LPA suggested it should swing freely and latch closed. 2 residents medications were checked against their mars at random, LPA asked the documentation for PRN dispersal process be updated: to include a status check of some kind after some amount of time from the drug being administered. LPA advised the documentation to also include start dates/dispersal dates on the punchout type medication blisters.
Fire extinguishers are dated 12/10/24. Smoke and CO alarms functional. Water temperature Measured 114*f. 1 staff interview, 1 client interview.
Client records 5 of 5 were reviewed. most recent admission agreement, ipp, medical evaluations were present and up to date. 2 of 2 staff records were reviewed, one of 2 had the first aid out of date, LPA gave guidance that staff member could not work a shift alone. Recent trainings, cpr/first aid, finger printing, criminal backround checks were otherwise present and up to date. Administrator documentation, including surety bond, LIC 500, administrator certificate, house rules, grievance policy, infection control plan, and evacuation control plan are present and up to date. No Citations issued as part of this visit, a copy of the report was read and given to the administrator.
