Facility Evaluation Report

Licensing Program Analyst(LPA) Noel Wolf Petersen arrived unannounced on 9/25/25 at 9:15 to conduct a Plan of Correction inspection at Michael's Residential Home Care for a deficiency that occured at another carehome with t he same owne r ( Facillity No. 397005036 ). LPA met with staff/administrator Victoria OGBUEHI-NZAMBI , and explained the purpose of the visit.

LPA conducted Physical inspection, including but not limited to bedrooms, bathrooms, kitchen, common areas, storage areas, exterior, and evacuation areas. Adequate food(2 days perishable, 7 days nonperishable, 3 days emergency) storage was observed, gas and water clearly connected, electricity functional.

LPA asked to review this months financial documents related to a previous office visit, 401(balance sheet), 403(Balance sheet suplemental), wages(payroll), food, utilties(garbage, water, electric, gas), surety bond, proof of ownership, liability insurance, workers comp. Files are physically present at the facillity in the phone of the owner, the facility will have a physical copy in a binder at a later date.

a copy of the report was read and given to the administrator. No citations issued in connection to the visit, the licensee is compliant with the plan of correction at this time.
