Facility Evaluation Report

On 4-23-2025 at 2:15pm, Licensing Program Analysts (LPAs) Michael Bilger and Noel Wolf Petersen arrived unannounced to conduct a case management visit. LPAs met with lead caregiver Nora Gardose and explained the purpose of the visit. Administrator Victoria Ogbuehi was notified via phone of LPA's visit and purpose. LPAs inspected the physical plant including but not limited to: Common areas, resident bedrooms, kitchen area, laundry area, and outside of facility to ensure compliance with Title 22 regulations. LPAs observed food supply to be in adequate amounts including meats, dairy, fruits and vegetables. Current census at facility is five. During observation, LPAs observed fire extinguisher last serviced on 11-27-2023.

As a result of today's case management, citation is cited under Title 22, Division 6 and noted on LIC 809D. An immediate civil penalty of $500 is issued in addition to citation due to violation of fire clearance and safety. An exit interview was conducted with lead caregiver and a copy of this report was provided. Appeal rights provided.
