Facility Evaluation Report
On 10-19-21 at 3:15pm, Licensing Program Analyst (LPA) Michael Bilger arrived unannounced to continue a case management visit opened on 10-1-21 regarding an incident which occurred on 8-27-21 involving Resident1 (R1) and Licensee. LPA met with Ilbefonso Letran, Administrator and explained the purpose of the visit. LPA conducted a case management inspection to ensure Title 22 compliance and health and safety concerns. All sharp objects and toxins were secured and inaccessible to clients in care. Room temperature was adequate during today’s visit. There are no obstructions to fire exits inside or outside facility. There was 1 staff on duty during visit. Based on additional interviews and records reviewed after the case management visit on 10-1-21 it is determined that R1 coordinated with service provider after the incident and made a decision to discharge to another destination on 9-9-21. It was further determined based on interview that R1 had made plans to discharge prior to the incident which occurred on 8-27-21.
Based on the interviews, inspection, and additional documentation received during this case management, the department has closed this case management and no further investigation is required. No deficiencies were cited as a result of today’s visit.
An exit interview was conducted with Ilbefonso Letran and a copy of this report was left with Ilbefonso.
