Facility Evaluation Report

On 9-21-21 at 2:07pm Licensing Program Analyst (LPA) made an unannounced case management visit to facility to deliver an addendum to a previously issued 9099 report dated 8-24-21 with complaint control number # 27-AS-20210824095221 . LPA met with Administrator Maricor Camero and explained the purpose of the visit. Addendum to complaint # 27-AS-20210824095221 now includes an immediate civil penalty in the amount of $500 as noted on LIC 421 due to absence of supervision on 8-20-21 in violation of section 80078(a).

An exit interview was conducted with Administrator and a copy of this report along with appeal rights was left with Administrator.
