Facility Evaluation Report

On 5/20/2025 at 2:00pm, Licensing Program Analyst (LPA) Michael Bilger arrived unannounced to conduct an annual inspection visit. LPA met with Licensee Maricor Junson and explained the purpose of the visit. At this time, facility remains empty of clients and is currently awaiting fire clearance and code enforcement clearance. Additionally, Licensee is seeking proof of building permit approval. Department sent information to fire department for clearance processing on 4-15-2025. Facility is not accepting clients at this time. LPA conducted a physical tour of facility during today's visit.

No citations issued as a result of today's visit. An exit interview was conducted with Licensee and a copy of this report was provided.
