Facility Evaluation Report
On 1/31/23 Licensing Program Analyst (LPA) Maja Jensen arrived at facility unannounced to conduct a case management visit related to an incident for an adverse event for resident 1 (R1). LPA Jensen met with Licensee Maryann Patacsil and explained the purpose of today's visit.
LPA Jensen requested and received copies of documentation from the file for R1. Document copies included but was not limited to Regional Center progress reports, identification, emergency contact information, physician's reports, medication data, charting notes, progress notes, and various medical records.
An exit interview was conducted and a copy of this report was given.
