Facility Evaluation Report

On 7-31-2026 at 3:30pm, Licensing Program Analyst (LPA) Michael Bilger arrived at facility unannounced to conduct a case management regarding a previous exemption denial for staff1 (S1). LPA met with Administrator Maryann Masaya and explained the purpose of the visit. LPA observed S1 not present during today's visit. Additionally, facility's LIC 500 does not indicate current employment of S1. LPA also observed a copy of S1's final pay stub for facility which was dated 7/30/2024. It is determined that S1 is not employed at facility.

No citations issued as a result of today's case management. An exit interview was conducted with Administrator and a copy of this report was provided.
