Facility Evaluation Report

On 9/24/25 at 1:00 PM, a meeting was held for Life Care Home about thier CHOL. In attendance are Brian Bennett, Robert Fernandez Jr, Katina Richardson, of VMRC, and Gurprit and Lakbir Singh, the Licensee's, as well as Licensing Program Manager LPM Liza King and Licensing Program Analyst LPA Noel Wolf Petersen.

Introductions then meeting commences. Paraphrased notes of the statements made by individuals are on an 812 from LPA Wolf Petersen associated to this office visit.

Meeting was called to address a possible concern that the licensee might lose control of the property.

LPM opens the meeting, asks for an update about the lease agreement because control of the property must be maintained to avoid forfeiting the license. Licensee provides that a month to month lease is secured. Licensee explains the delay with the Change of Location application was unintentional, that they had started this application in April of 2024 and when it came time for the 9054 fire inspection form to be processed the Cab analyst took a month of vacation. VMRC asks for the service coordinator and planning team to be looped in to further updates as to the situation. CCL asks for everyone to be looped in to further updates. Licensee is to submit that evidence of the secured lease to the department by end of day 9/24/25, a lease agreement and letter the licensee has in their possession at the time of the meeting.

No further questions from the department or vmrc.
