Facility Evaluation Report

On 05/26/2026, Licensing Program Analyst (LPA) Albert Johnson arrived unannounced to this facility to conduct a case management visit.

LPA was following -up on plan of correction for the repairs of the facility for fire clearance violation on 1/2/2026. LPA toured the facility and interviewed staff about sleeping arrangements. The sleeping area for the staff is not completed.

The facility has to revise the plans due to some additional requirements and will send copies of the revised plans/permits to the department by the close of business on 5/29/2026.

The facility is anticipating that the modification will be completed in approximately two weeks.

No other deficiencies cited. Advisories given for construction area safety awareness.
