Facility Evaluation Report

Licensing Program Analyst (LPA) Albert Johnson arrived unannounced to conduct an plan of correction visit. LPA met with Administrator Rendento Magsayo.

LPA was following -up on an annual inspection that was completed on 1/2/2026, by the department. The facility was given citations and proved the department with correction for those citations. LPA requested that the facility forward the plans to verify that they have been completed. Additional, citations given before the visit on 1/2/2026 were cleared today.

The following deficiencies, initially cited during a visit on 04/25/2024,

Plan of Correction: Staff secured dangerous items during LPA visit.

Licensee submitted a plan ensuring on-going inaccessibility of chemicals, sharp objects, and other dangerous objects. Plan to be submitted to LPA by POC due date. Corrections:
