The inspector’s account

On 8/8/25 at 10:30am, licensing program analyst( LPA) Noel Wolf Petersen arrived unanounced and met with backup administrator edwin valdez and explained the purpose of the visit. J & R Magsayo Home II is licensed for 6 adults ages 18-59. The current census is 6, two of the clients are older than 60 and a third will be over 60 next november.

The Physical plant was inspected, including but not limited to the kitchen, storage areas, common areas, resident bathrooms, resident bedrooms, the exteriors and the evacuation routes. The facility is clean and the traffic areas are well lit and unobstructed. The kitchen has lockable storage for sharps and toxics, the food has 2 days of perishable and 7 days of nonpershible supplies. The common areas are in good repair and have room for activies, the fire extinguisher is dated 8/21/2025. Bathrooms have hardware in good repair, water temperature was measures within the 105 to 120 degree range. exteriors have room for activities and have no trip hazzards. Evacuation route gates are in need of some repair, LPA observed that while openable they drag into the ground and could present an obsticle more than an aid in the event of an emergency. The back up administrator provided that they have a second gate, and the offending gate would rehung. MARs and Medications were checked for 2 clients at random, they were found to be in accordance with the record in quantitiy and kind. First aid kit has all necessary items. Fire drill log is up to date.

No clients were present at the time of the visit, all were in day program, 0 client interviews. 1 staff was able to be interviewed.
