This is an official report of an unannounced visit/investigation of a complaint received in our office on

04/05/2024 and conducted by Evaluator Kesha Lewis

COMPLAINT CONTROL NUMBER: 27-AS-20240405131303

Staff handled resident in a rough manner.

Licensing Program Analyst (LPA) Kesha Lewis arrived unannounced to deliver findings on this complaint investigation. LPA Lewis met with facility staff and explained the purpose of the visit.

Based on the interviews and statements obtained during the investigation process, the allegations are

Allegation 1: Staff handled resident in a rough manner ard residents. Allegation 2: Staff yell at residents are found to be UNSUBSTANTIATED. Based on LPA'S observation and interviews with staff and residents nighter comfiremed the allegations were ocuorring. The Department had investagated the above allegations and they are found to be UNSUBSTANTIATED, which means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. No deficiencies were cited regarding the above allegation.
