Facility Evaluation Report

On 12-11-2025 at 9:00am, a meeting was held via Microsoft teams to review the facility's compliance with the on-going quarterly visits. Present at the meeting were Licensing Program Manager (LPM) Liza King, Licensing Program Analyst (LPA) Michael Bilger, Regional Center representative Brian Bennett, and Ombudsman Kathryn Thomas. License Lourdes Fernandez was also present. An informal meeting was held with licensee on 11-5-2024 and a compliance plan was developed to address adherence to personal rights of residents, on-going compliance with updated resident assessments, staff availability to meet resident needs, and Administrator presence at facility at least 40 hours per week. The department conducted quarterly visits to evaluate compliance progress with last visit conducted on 11-6-2025. Additionally, LPM and LPA discuss age exception requirements with licensee which includes but are not limited to: Medical assessment updates, appraisal form updates, and various training requirements. Licensee was made aware to review exception request and notify LPA for any questions and clarification. Licensee also stated during today’s meeting that she may consider a conversion to residential care facility for the elderly (RCFE) in the future and so inform regional center and the Department of her intentions.

At this time the department finds the facility to be in compliance with the above, and the department has no concerns regarding the facility at this time, therefore the quarterly visits will cease.
