Facility Evaluation Report

A Technical Support Program (TSP) engagement visit was conducted today on 4/24/25 via Microsoft Teams with the Sacramento South Regional Office. Present at today's meeting include the following: Licensing Program Manager(LPM) Liza King, and Licensing Program Analyst(LPA) Noel Wolf Petersen, LPA Michael Bilger; Regional Center Represent ivies: Katina Richinson, Brian Bennett, Chris Pillsbury; Facility representatives: Lourdes C. Fernandez; and local ombudsman: Kathyn Thomas.

The TSP process was explained during this meeting. In addition, TSP report findings were delivered during this Microsoft Teams meeting.

Topics discussed during the TSP Engagement Meeting:

· Facility representatives have implemented plans and procedures to support their compliance efforts. The power points were reviewed in an in-service, and they are printed in a binder able to be reviewed by staff. All of the staff are reviewing the IPPs, and the licensee is documenting current and future training by HCBS for review by CCL during the quarterly visits.

· Facility representatives have implemented plans and procedures and tools to train facility staff (personal rights tool, self assessment guide). Content of the personal rights training should have relevant examples for the staff to better grasp the guidance in the training. Licensee will provide a updated LIC 500 for the purpose of scheduling.

Per California Code of Regulations (CCRs) - Title 22 no deficiencies are being cited at this visit. An exit interview was conducted with facility staff, and a copy of this report was provided via email and an electronic email read receipt confirms receiving these documents.
