Facility Evaluation Report

Licensing Program Analyst (LPA) Albert Johnson conducted an unannounced Annual visit on this date.

A tour of the facility was made The interior and the exterior of the facility were inspected including the common living spaces, resident bedrooms and bathrooms, and kitchen. Medication storage was checked. There no residents in care.

LPA observed the facility to be free of odor, clean and in good repair. LPA observed sufficient furniture and lighting throughout the facility. There are no bodies of water present in/or around the facility.

LPA observed sufficient seven day non-perishable food supply. Hot water temperature was measured at 118 degrees Fahrenheit in client bathroom sink.

Per California Code of Regulations, Title 22 Division 6, Chapter 8, no deficiencies were observed during this visit.
