Facility Evaluation Report
Licensing Program Analyst (LPA) Noel Wolf Petersen arrived unannounced to conduct annual inspection, LPA Met with the administrator Kathleen Quiot and explained the purpose of the visit. Graceful Living for seniors is a 5 capacity facility, although probably will limit its capacity to 4 at VMRC's request. No Clients are currently in care, awaiting vendorization with VMRC.
The Physical Plant was inspected including the kitchen, common areas, bedrooms, bathrooms, exteriors, storage, and evacuation routes. The facility is clean and the traffic areas are well lit. The kitchen has lockable storage for medications, and sharps. The facility does have 7 days of nonperishable food, but doesnt have 2 days of perishable food. LPA acknowledged there are no clients in care, but provided guidance that before a client enters the building that should be remedied. The common areas have enough room for activities, the bedrooms have all required furniture and furnishings, The administrator would like to change the lay out from 3 bedrooms and a staff room serving 5 clients to 4 bedrooms serving 4 clients. LPA gave guidance to send in the new facility sketch and a LIC200, then get the fire department to do a new inspection. Bathrooms have a hardware problem in one of the showers, deliverable water is within 105-120*f. Exteriors are clean, have activity space. evacuation routes either need their posts shaved or the gates rehung to facilitate rapid egress during a evacuation. Administrator will send a picture when the gates can swing freely and latch closed. The fire extinguisher is dated 1/16/24, the LPA pointed this out to the administrator, she was receptive to picking up a new one at the hardware store and sending a picture of the receipt to the LPA. The first aid kit has all required items. There are no clients, no client interviews. There are no staff, no staff interviews. Review of client and staff documents will be completed with the Post Licensing.
Administrator documents will be emailed to the LPA: control of property, and liability insurance, and training's for hiv, firstaid/cpr, and administrator certificate. In the future, documents for the facility will be kept here at the facility. no citations were issued as part of this visit. An Exit interview was conducted. A copy of the report was read and given to Administrator.
