Facility Evaluation Report
Licensing Program Analyst Albert Johnson conducted an unannounced case management visit to the facility to follow up on an incident.
LPA reviewed special incident report dated 10/31/2025 stating R1 was choking on his own saliva. It is unknown if recovered from the incident. R1 has not returned to program. R1 physician's report confirmed that there is a food restrictions and a special instructions or health care plans for swallowing.
The facility was not provided with discharge papers; because R1 has not returned to program.
During today's visit, LPA reviewed R1's file, medical records related to the incident and service plan including R1's individual program plan (IPP).
