Facility Evaluation Report
Licensing Program Analyst(LPA) Noel Wolf Petersen arrived unannounced to conduct a annual inspection at 1:30 on 8/20/25. The LPA met with the administrator JJ Jose to explain the purpose of the visit. Estrella's care home No. 3 is a 6 capacity Adult residential Facility for clients aged 18 to 59 level 4i/6, the current census is 6.
The physical plant was inspected including but not limited to the kitchen, common areas, client bedrooms, client bathrooms, the storage areas, the exteriors, and evacuation routes. the Facility was clean, and traffic areas are well lit and unobstructed. Kitchen has adquate storage for sharps, medication, and toxics. Food supply is adequate for 2days perishable and 7 days nonperishable and 3 days emergency foods. Common areas are well equipped for actives. Bedrooms have required furniture's and furnishings, lacks bed bug encasements. Administrator was open to getting them. Bathrooms have hardware in good repair, missing a shower mat in one bathroom. Administrator was open to getting one. The exteriors are clean and have adequate space. Evacuation route exterior gate is secured with a bit of cord and dragging against the ground, the administrator will ajust/fix that gate such that it can swing freely and self close. posters are of correct dimensions, fire extinguishers are last dated 7/29/25.
Administrator file contained present and up-date- surety bond, liability insurance, facility sketch. 5 of 6 client files were reviewed. they contained present and up-to-date: fingerprinting clearance, first aid, and trainings. 5 of 12 staff files were reviewed. they contained present and up-to-date, 602s, IPPs, preapraisals for health and mental health, P+I, safe gaurded property, and signed admission agreements. 0 interviews for clients, they were nonverbal, seemed happy, clean, well fed. 2 staff were interviewed.
Exit interview was conducted. a copy of the report was read and left with the staff.
